If you are looking to enrol your child at St Francis Xavier Primary School, please download and then complete the below enrolment form and return it to the school office at your earliest convenience.
Forms can be handed to the office directly or can be scanned and emailed to office@sfxfrankston.catholic.edu.au.
A copy of your child’s Birth Certificate, Immunisation Certificate and Baptism Certificate (if applicable) must be submitted with the enrolment form.
